dc.description.abstract This report examines the PASSPORT assessment and services processes through thirty case studies: ten new assessments; ten ongoing consumers; and ten disenrolled consumers. We used a combination of observation; interviews with consumers, family caregivers, assessors, and case managers; consumer records review; and one focus group of PASSPORT Administrative site directors or their designees. We evaluated the accuracy and comprehensiveness of assessments; the appropriateness of service plans; and the extent to which consumers and their family members made informed decisions about their care and services. en_US
